Medical Records Release
If you need to see a provider outside of think, you’ll need to fill out the Medical Records Release form and send the form to:

Psychotherapy Notes Release
Psychiatric notes require a different release form in order for Think’s Medical Records team to send them to your designated recipient. Complete the release form and send it to:

Consent to Photograph
We love sharing our patients’ stories, but want to protect your privacy. When taking photos in the building, we are careful to keep patients unrecognizable unless they give permission to be filmed. We ask every patient or visitor who agrees to be photographed or filmed to complete our consent form.
